Choosing a Proton Therapy Center


At this writing, there are 41 proton therapy centers operating in the United States with 22 more in development. That is pathetically few. Given the promise of proton therapy, centers should be far more abundant. The number of potential clients exceeds the current capacity by a multiple of ten, meaning there are sufficient numbers of cancer cases to keep 400 centers busy. What’s wrong? The progress of proton therapy has been disappointing for reasons I discuss in a different essay (see: ISSUES)

The above circumstances aside, you are likely reading this to find the best proton therapy center for you. Here are some factors to consider.

I previously had a website that listed all proton therapy centers. I decided in this essay to give a link to other sources for those centers. Unfortunately, even the National Association of Proton Therapy (NAPT) list was not complete, partly because they list their members and not all centers are members. They do, however, have a very nice map showing the location of most of the centers. So, at the end of this essay, I have given a link to NAPT but have also included my list of centers.

Location may be a consideration, although if you do not live near enough to commute to a nearby proton center, it seems to make little difference if you travel one state away or go across the country. In any event, you will incur costs for food and lodging.

That being said, you may be willing to travel further if it involves a substantial savings. Not all centers charge the same, even for the same treatment and basically the same equipment. As I have mentioned elsewhere (see: EXPERIENCE), I found the same 39-session treatment for prostate cancer cost $134,000 in Florida and $93,000 in Tennessee. There are at least four centers (the two Mayo Clinic proton centers, and the universities of Pennsylvania and Maryland) that charge the same for proton therapy as for photons, i.e., x-rays. This should be considerably less. A lower cost could save you money if paying out of pocket, or might help to convince your insurance company to cover the treatment, since they likely already cover x-rays and there is no difference in price.

Location and cost are relevant if considering going outside of the United States. For example, the proton therapy center in Prague, in the Czech Republic, charges about a third of what proton therapy costs in the United States. That means you could go there, have a nice vacation, and still come out ahead. And they aren’t the only ones. Other countries, such as India, Korea, and China, offer proton therapy at reduced prices. There are centers in Europe and the United Kingdom, but I have no feel for the cost.

Twice I convinced my insurance company to cover out-of-country medical treatments, once for my wife’s breast cancer treatment in Germany and once for my three-day stay (pneumonia) in a Mexican hospital (Puerto Vallarta, and the experience was stellar). In both cases, I pointed out that the cost in the United States would have been far higher. Perhaps one could make this arrangement before getting the treatment, rather than convincing them after the fact as I did. Given the costs of medical care, I think insurance companies should charter medical flights to Thailand or elsewhere.

Most proton centers treat most cancers. There could be some reason, however, to choose one center over another because of their special expertise. Pediatric cancers, for example, might lead one to St. Jude's Children's Hospital in Memphis (strangely called Red Frog Events Proton Therapy Center, after its principle donor), or Children’s Hospital in Cincinnati. Perhaps one will have a Ronald McDonald House nearby that would provide lodging or other services. Proton centers list on their websites what cancers they cover. You might also choose a center because it is conducting a clinical trial that would be of significance for your situation. (See: www.clinicaltrials.gov) Some centers are located in very large hospitals that offer many other services, such as MD Anderson or the Mayo Clinic.

Differences in technology inspire me to suggest you go to a center that has opened within the last three years. The original technology, in 1990, involved a conformal system that spreads out proton beam and then shapes it to the contour of the tumor. This is called double scattering. Many thousands of people have benefited from this original technology. But now, technological advances lead to far more accurate precision. The newer technology (see PHYSICS), called pencil beam scanning (PBS), lays down tiny spots in layers and rows, covering the tumor exactly, plus a tiny margin to be sure.

Older centers that had double scattering have mostly updated to have PBS, but they still have the older technology and still use it. That’s why I suggest newer centers. A further advancement is called Intensity Modified Proton Therapy IMPT), which has a counterpart in x-rays called Intensity Modified Radiation Therapy (IMRT), the most accurate procedure for delivering X-rays. I saw a video by a prominent prostate cancer radiologist who still believes IMPT and IMRT are essentially equivalent. I don’t think that’s the case (see COMPARISON).

Advances in imaging have accompanied and enabled those of proton therapy. For example, cone beam computed tomography gives a 3-D version of the target area. There are developments in proton computed tomography, as well as live imaging that takes place during the actual treatment. Again, the newest centers are more likely to offer these technologies.

A considerable variation exists in the treatment protocols of different proton centers. Take prostate cancer, for example. My treatment involved low doses of proton radiation over a seven-week period, thirty-nine all told. However, some centers are using fewer treatments at higher doses with similar results. I could have gone to a center that offered only five sessions (called fractions). That would mean less time away from home, lower cost, and other benefits.

I personally chose to avoid hormone therapy during my treatment for prostate cancer, even though it commonly accompanies or precedes proton therapy. For some cancers, they combine more than one modality, such as chemotherapy and radiation, or radioactive seeds and proton therapy. Such combinations seem to offer a higher success rate in eliminating the cancer, although I don’t know about the side effects. Sometimes, surgery may precede proton therapy.

Some proton centers operate at 70-80% capacity, maybe even less. They could likely begin treatment immediately after the prescribed preparation. Other centers, especially the more famous ones, may have a waiting period before treatment is available. If timing is important, be sure to ask when you could begin.

Most centers have deep experience dealing with insurance companies and wheedling coverage from them. However, cost or location or expertise by the insurance department could make a difference in whether you obtain insurance coverage. Proton centers rightfully require assurance of payment prior to treatment, whether insurance or out of pocket.

So there you have a number of considerations when choosing a proton therapy center. Hopefully, you will find exactly the right fit for you. Below are some links and then my current list of proton centers.
